PILGRIM PLACE HEALTH SERVICES CENTER (HSC) IS CLOSED TO ALL VISITORS
Pilgrim Place's skilled nursing facility (HSC) has been following the guidance offered by the Centers for Disease Control (CDC) and the Centers for Medicare and Medicaid Services (CMS) regarding the coronavirus (COVID-19). At this time the guidance being issued by CMS is to restrict all visitation and non-essential healthcare personnel, except for certain compassionate care situations, such as an end of life situation. We do not know how long this restriction will be in place. If you have a family member residing in the Pilgrim Place Health Services Center please call the central nurses station at (909) 399-5537 for information about your loved one. Please, this number is for family member or responsible party information ONLY!
